What is your financial policy?

Payment is expected at the time of service unless other arrangements have been made. Payment may be made by cash, check, money-order, MasterCard, Visa, Discover or American Express.


What insurance do you accept?

We participate with Traditional Medicare and several PPO organizations. Please call us to obtain a current list of the participating carriers.


How do I get a refill on my prescription?

Whenever possible, prescriptions renewals should be called in two days before your supply is exhausted. Please have the telephone number of your pharmacy available. All prescription refills must be called in during working hours of 8:00AM to 5:00 PM.


What are your office hours?

We are open between 8:00AM to 5:00 PM on Monday through Friday. We have coverage 7 days a week 24 hours a day for emergencies.


What should I bring to my appointment?

Be sure to bring your insurance card, referral if needed, and workers compensation authorization if needed. Also bring any previous doctor notes, x-rays, MRI Films and/or surgery pictures.
